Diabetes Management Plan
The Liverpool Central School District is committed to providing a safe and nurturing environment for students.
All children with diabetes should have a Diabetes Management Plan that is completed by the student’s personal health care team and parents/guardian. This plan should outline the child's diabetes treatment - target blood glucose range, insulin schedule, eating plan, and usual blood glucose testing times. It should also include instructions on what to do in various situations (treatment for hypoglycemia, for example).
Parents/guardians are responsible for getting their child's completed and signed Diabetes Medical Management Plan (or doctor's orders) from their child's diabetes care provider.
